Add Education Organization/Training Provider Form

Description: Welcome to the Education Management page. This page displays the Add Education Organization/Training Provider form. This form allows users the capability to Add a New Education Organization/Training Provider record to the YouthBuild system.

Complete the following to Add Education Organization/Training Provider:

Fill out all required fields on this form.

In the Institution/Organization Name text box, enter the name of the institution/organization. (Required Filed)
In the POC First Name text box, enter the point of contact's first name. (Required Filed)
In the POC Last Name text box, enter the point of contact's last name.
In the Organization/Provider Address 1 text box, enter the provider's street address.
In the Organization/Provider Address 2 text box, enter the provider's remaining address information (if needed).
In the Organization/Provider City text box, enter the city where the provider is located.
Using the State drop-down list, select the state where the provider is located.
In the Zip text box, enter the zip code where the provider is located.
In the POC Phone text box, enter the point of contact's phone number. (nnn-nnn-nnnn)
In the POC Email text box, enter the point if contact's email address.

Once all required fields have been completed, click the (Save) button. This will display the Successfully Saved Message.

Click the 'here' link to return to the Education Management page. The newly created Institution/Organization Provider will display in the Institution/Organization Management Overview section.
