Add New Employment Form   

Description: Welcome to the Outcomes page. This page displays the Add Employment form. This form allows users the capability to Add a New Employment Placement record to the participant's case.  

Functions: This page contains the following functions:

• Add New Employment Placement

• Add Employer

Add New Employment Placement

Complete the following to Add a New Employment Placement record:

• Fill out all required fields on this form.

• In the Date of Placement text box, enter the date the participant was placed at a job. Users can also use the (Calendar) icon to select the date. (Required Filed)

• Using the Occupation at Placement drop-down list, select the participant’s occupation at the time of placement. (Required Field)

• In the Occupational Title text box, enter the participant’s title.

• Using the Employer Name drop-down list, select the employer’s name. (Required Field)

Note: If the Employer’s name is not in the Employer Name drop-down list, users must Add the Employer to the system before Adding the Employment record. To Add a New Employer, click the Add Employer link located next to the Employer Name text box. If the 'Add Employer' link is selected all information entered on this form will be erased.

• In the Last Date of Employment text box, enter the last date the participant was employed. Users can also use the (Calendar) icon to select the date.

• In the Hourly Wage text box, enter the participant’s current hourly wage. (Required Field)

• In the Number of Hours Worked During the 1st Full Week text box, enter the number of hours the participant worked the first full week. (Required Field)

• In the Reason for Leaving Placement in Unsubsidized Employment text box, enter the participant’s reason for leaving the unsubsidized employment.

• Once all required fields have been completed, click the (Save) button. This will display the Successfully Saved Message.

• Click the 'here' link to return to the Outcomes page. The newly created Employment record will display in the Outcomes Overview section.
