Deletion Request Form  

Description: Welcome to the Deletion Request Form page. This page displays the Delete Request form. This form allows users the ability to submit a deletion request on the selected case to the National Office. Users must complete all required fields prior to submitting the deletion request. Complete the following to confirm the deletion request:

  • In the Requester's First Name text box, enter the requester's first name. (Required Field)

  • In the Requester's Last Name text box, enter the requester's last name.  (Required Field)

  • The Request Date text box will be pre-populated with the system date.

  • In the Requester's Email text box, enter the requester's email address.  (Required Field)

  • In the Reason for Delete text box, enter a descriptive reason why the selected case should be deleted.  (Required Field)

  • Once all required fields have been entered, click the (Confirm Request) button. Users will be directed back to the Summary page.

  • Users can click the (Reset) button to reset the Delete Request form or the (Cancel) button to cancel the deletion request and return to the Summary page.
