Deletion Request Form  

Description: Welcome to the Deletion Request Form page. This page displays the Delete Request form. This form allows users the ability to resubmit a deletion request if it has been rejected by the National Office. The National Office will have provided comments back explaining why the deletion request was rejected. Those comments can be found in the Delete Request History section located below the Delete Request Form. Complete the following to Resubmit the deletion request:

  • In the Comments text box, enter a descriptive reason why the selected case should be deleted.  (Required Field)

Note: 1) This field is ONLY required if users are resubmitting the deletion request. 2) Review the comments sent by the National Office to ensure you respond back with all necessary information. Comments can be found in the Delete Request History section located at the bottom of the page. The Delete Request History section is displayed with the most current information on top.

  • Once all required fields have been entered, click the (Resubmit Request) button. Users will be directed back to the Summary page.

To reset the Deletion Request, complete the following:

  • Users can click the (Reset Request) button.

Note: All information that has been submitted to the National Office will be gone.

  • A warning message will appear stating: "Are you sure you want to reset the Deletion Request? All information will be erased and the form will be reset completely. Click the Cancel button to return to the form."

  • Click the (Ok) button. This will reset the participant's Deletion Status to New.

To return to the Summary page, complete the following:

  • Users can click the (Cancel) button to return to the Summary page.
