Selection: Select one of the

Re-adjudicated (Note: This only counts towards the
recidivism rate if it occurs within 12 months of program enrollment.)
The following describes how to complete the
specific fields that display on the Add
Arrest/Re-adjudication/Re-incarceration form when the "Re-adjudicated (Note: This only counts towards
the recidivism rate if it occurs within 12 months of program enrollment.)"
radio button is selected from the Arrest/Re-adjudicated/Re-incarceration
Selection form.
In the Additional
Information section, complete the following:
Using the Most
Serious Charge for New Crime drop-down list, select the most serious
charge given to the participant for the crime committed.
Select one of the Convicted
of New Crime radio buttons to identify if the participant was convicted
of a new crime.
In the Date
Convicted of New Crime text box, enter the date the participant
was convicted of the new crime. Users can also use the (Calendar) icon to select the date.
Select one of the Incarcerated
After New Conviction radio buttons to identify if the participant
was incarcerated after the conviction.
In the Date
Incarcerated After New Conviction text box, enter the date the
participant was incarcerated after the conviction. Users can also use
the (Calendar)
icon to select the date.
In the Date
Released from Incarceration for New Crime text box, enter the date
the participant was released from incarceration. Users can also use the
(Calendar) icon
to select the date.
