Update Arrest/Re-adjudication/Re-incarceration Form

Description: Welcome to the Outcomes page. This page displays the Arrest/Re-adjudication/Re-incarceration form. The fields displayed on this form will vary depending on the type of Arrest/Re-adjudication/Re-incarceration record that was created. This page provides the users with the capability to Update the Arrest/Re-adjudication/Re-incarceration form.  

 

Selection: Select one of the following links for step by step instructions on completing the specific fields:

 

Re-adjudicated (Note: This only counts towards the recidivism rate if it occurs within 12 months of program enrollment.)

The following describes how to complete the specific fields that display on the Add Arrest/Re-adjudication/Re-incarceration form when the "Re-adjudicated (Note: This only counts towards the recidivism rate if it occurs within 12 months of program enrollment.)" radio button is selected from the Arrest/Re-adjudicated/Re-incarceration Selection form.

  • Fill out all required fields on this form.

    • In the Date Re-adjudicated text box, enter the date the participant was re-adjudicated.  Users can also use the image91.gif (Calendar) icon to select the date. (Required Field)

    • In the Reason for Arrest/Adjudication text box, enter the reason for the arrest/adjudication.

    • Using the Type of Conviction drop-down list, select the type of conviction.

    • In the If 'Other', please specify text box, enter the applicable type of conviction. (Required if Other is selected from the Type of Conviction drop-down list)

In the Additional Information section, complete the following:

    • Using the Most Serious Charge for New Crime drop-down list, select the most serious charge given to the participant for the crime committed.

    • Select one of the Convicted of New Crime radio buttons to identify if the participant was convicted of a new crime.

    • In the Date Convicted of New Crime text box, enter the date the participant was convicted of the new crime. Users can also use the image91.gif (Calendar) icon to select the date.

    • Select one of the Incarcerated After New Conviction radio buttons to identify if the participant was incarcerated after the conviction.

    • In the Date Incarcerated After New Conviction text box, enter the date the participant was incarcerated after the conviction. Users can also use the image91.gif (Calendar) icon to select the date.

    • In the Date Released from Incarceration for New Crime text box, enter the date the participant was released from incarceration. Users can also use the image91.gif (Calendar) icon to select the date.  

  • Once all required fields have been completed, click the image92.gif (Save) button. This will display the Successfully Saved Message.

  • Click the 'here' link to return to the Outcomes page. The updated Arrest/Re-adjudication record will display in the Outcomes Overview section.

  • Fill out all required fields on this form.

    • In the Reason for Arrest/Adjudication text box, enter the reason for the arrest/adjudication.

Re-incarcerated for a revocation of the parole or probation order for violations of terms of sentence

The following describes how to complete the specific fields that display on the Add Arrest/Re-adjudicated/Re-incarceration form when the "Re-incarcerated for a revocation of the parole or probation order for violations of terms of sentence" radio button is selected from the Arrest/Re-adjudication/Re-incarceration Selection form.

  • Fill out all required fields on this form.

    • In the Date Re-incarcerated text box, enter the date the participant was re-incarcerated.  Users can also use the image91.gif (Calendar) icon to select the date. (Required Field)

    • In the Reason for Arrest/Adjudicated text box, enter the reason for the arrest/adjudication.

    • Using the Type of Conviction drop-down list, select the type of conviction.

  • Fill out all required fields on this form.

    • In the Date of Violation text box, enter the date of the violation. Users can also use the image91.gif (Calendar) icon to select the date.  (Required Field)

    • In the Reason for Arrest/Adjudication text box, enter the reason for the arrest/adjudication.

  • Once all required fields have been completed, click the Save button. This will display the Successfully Saved Message.

    • In the Date Arrested text box, enter the date the participant was arrested/incarcerated.  Users can also use the image91.gif (Calendar) icon to select the date. (Required Field)

    • In the Reason for Arrest/Adjudication text box, enter the reason for the arrest/adjudication.

    • Using the Type of Conviction drop-down list, select the type of conviction.

    • In the If 'Other', please specify text box, enter the applicable type of conviction. (Required if Other is selected from the Type of Conviction drop-down list)

The following describes how to complete the specific fields that display on the Add Arrest/Re-adjudication/Re-incarceration form when the "Arrested but charges dropped (Note: This option does not count towards the recidivism rate.)" radio button is selected from the Arrest/Re-adjudication/Re-incarceration Selection form.

 

  • Fill out all required fields on this form.

    • In the Date Arrested text box, enter the date the participant was arrested/incarcerated.  Users can also use the image91.gif (Calendar) icon to select the date. (Required Field)

In the Additional Information section, complete the following:

    • Using the Most Serious Charge for New Crime drop-down list, select the most serious charge given to the participant for the crime committed.

    • In the Comments text area, enter applicable comments that pertain to the record being created.

  • Once all required fields have been completed, click the image92.gif (Save) button. This will display the Successfully Saved Message.
