Add New IDP Form

Description: Welcome to the IDP page. This page displays the Add New IDP form. This form allows users to Add a New Individual Development Plan (IDP) to a participant's case.

Add a New IDP

Complete the following to Add a New IDP:

  • Fill out all Required fields to Add a New IDP.

    • Using the IDP Goal drop-down list, select the desired IDP type. (Required Field)

    • In the IDP Description text box, enter a brief description of the IDP. This field is limited to 400 Characters.  (Required Field)

    • In the Date to be Attained text box, enter the projected date the IDP will be completed by. Users can also use the image105.gif (Calendar) icon to select the date. (Required Field)

    • In the Date Actual Attained text box, enter the actual date the IDP was attained. Users can also use the image105.gif (Calendar) icon to select the date.

    • In the IDP Evidence text box, enter the desired IDP Evidence.

  • Once all required fields have been completed, click the image40.gif (Save) button. This will display the Successfully Saved Message.

  • Click the 'here' link to return to the IDP page. The newly created IDP record will display under the Individual Development Plan (IDP) section.
