Add New Educational Placement Form

Description: Welcome to the Outcomes page. This page displays the Add Educational Placement form. This form allows users the capability to Add a New Educational Placement record to the participant's case.

Add New Educational Placement

Complete the following to Add a New Educational Placement record:

Fill out all required fields on this form.

In the Date of Post-Secondary Education or Training Placement text box, enter the date the participant was placed in the post-secondary education or training. Users can also use the (Calendar) icon to select the date. (Required Filed)
Using the Institution/Organization Name drop-down list, select the institution/organization providing the education or training. (Required Field)

Note: If the Institution/Organization's name is not in the Institution/Organization Name drop-down list, users must Add the Institution/Organization to the system before Adding the Educational Placement record. To Add a New Institution/Organization, click the Add Institution/Organization link located next to the Institution/Organization Name text box. If the 'Add Institution/Organization' link is selected all information entered on this form will be erased.

In the Major Focus text box, enter the participant's primary focus for the education/training.
In the Degree Expected text box, enter the type or name of degree expected.
Using the Type of Program drop-down list, select the type of program the participant has been enrolled. (Required Field)
In the If 'Other' please specify text box, enter the type of program the participant has been enrolled. (Required if the 'Other' option was selected from the Type of Program drop-down list.)
In the Last Date of Education text box, enter the participant's last date of participation. Users can also use the (Calendar) icon to select the date.
Select one of the Full-Time radio buttons to indicate if the participant is full-time or not.
In the Hourly Training Wage $ text box, enter the participant's hourly training wage if applicable. (Format: xx.xx)

Once all required fields have been completed, click the (Save) button. This will display the Successfully Saved Message.

Click the 'here' link to return to the Outcomes page. The newly created Educational record will display in the Outcomes Overview section.
