Add Post Test Form

Description: Welcome to the Basic Skills page. This page displays the Add New Post-Test Score form. This form allows users the capability to Add a Post-Test. The Post-Test is administered after the initial pre-test. This will allow Case Managers the ability to view the participants progress.

Add a Post-Test Score

Complete the following to Add a Post-Test score to the participant's case:

• Fill out all required fields on this form to add the post-test results.

• In the Date Administered Post-test text box, enter the date the Post-Test was administered. Users can also use the (Calendar) icon to select the date. (Required Field)

• Using the Educational Functioning Level drop-down list, select the specific educational functioning level. (Required Field)

Note: Located on the top of the Add New Post-Test Score form is the 'EFL Descriptor Levels' link. The browser will open a pdf file in a new window which will contain the Educational Functioning Descriptor Levels. This will information assist users in selecting the correct Educational Functioning Level based on the participants test score.

• In the Post-test Score text box, enter the participant’s Post-Test score. (Required Filed)

• Once all required fields have been completed, click the button. Users will be directed back to the Update Basic Skills form. The Post-Test information will be displayed below the pre-test information.  

• Once all required fields have been completed, click the (Save) button. This will display the Successfully Saved Message.

• Click the 'here' link to return to the Basic Skills page. The newly added Post-Test score will be added to the Basic Skills test.
