Description: Welcome to the Basic Skills page. This page displays the Add Basic Skills Test form. This form allows users the capability to Add a New Basic Skills test to the participant's case.

Add a New Basic Skills Test

Complete the following to Add a New Basic Skills Test:

  • Fill out all required fields on this form to Add a New Basic Skills Test.  

    • Select one of the Category of Assessment radio buttons. (Required Field)

    • Using the Type of Assessment Test drop-down list, select the type of assessment test taken by the participant. If the type of assessment test taken by the participant is not fount in the Type of Assessment Test drop-down list, users must select the ‘Other’ option (Required Field)

    • If the ‘Other’ option was selected, users MUST enter the type of assessment taken in the Other Assessment text box. (Required ONLY if the ‘Other’ option was selected)

    • Using the Functional Area drop-down list, select the specific functional area the test was taken for. (Required Field)

    • In the Date Administered Pre-test text box, enter the date the Pre-Test was administered. Users can also use the image140.gif (Calendar) icon to select the date. (Required Field)

    • In the Pre-test Score text box, enter the participant’s Pre-Test score. (Required Field)

    • Using the Educational Functioning Level drop-down list, select the participant's educational functioning level. (Required Field)

Note: Located on the top of the Add Basic Skills Test form is the 'EFL Descriptor Levels' link. The browser will open a pdf file in a new window which will contain the Educational Functioning Descriptor Levels. This will information assist users in selecting the correct Educational Functioning Level based on the participants test score.

  • Once all required fields have been completed, click the image38.gif (Save) button. This will display the Successfully Saved Message.

  • Click the 'here' link to return to the Basic Skills page. The newly created Pre-Test record will display in the Basic Skills Overview section.
