Case Overview
Description: Welcome to the
Case Overview page. This page
displays the Add a New Case form.
This form is used to Add a participant's Case Information to the YouthBuild
system.

Note:
If the Other check box is selected,
users must insert the other funding source that supports the participant
in the Other text box.
Using the Cohort
drop-down list, select a Cohort. (Required
field)
Using the Team
drop-down list, select a Team.
In the First
Name text box, enter the participants first name. (Required
field)
In the Middle
Initial text box, enter the participants middle initial.
In the Last
Name text box, enter the participants last name. (Required
field)
In the Address
1 text box, enter the participants street address. (Required
field)
In the Address
2 test box, enter the participants remaining address information.
In the City
text box, enter the participants city. (Required
field)
Using the State
drop-down list, select the participants state. (Required
field)
In the Zip
text box, enter the participants zip code.
In the Phone
text box, enter the participants main phone number. (999-999-9999)
(Required field)
In the Phone
2 text box, enter an additional phone number for the participant.
(999-999-9999)
In the Other
Phone text box, enter an additional phone number for the participant.
(999-999-9999)
In the Two
Way Pager Number text box, enter the participant's pager number.
This field has a maximum character entry of 13.
In the Email
text box, enter the participants email address.
In the Date
of Birth text box, enter the participants date of birth. (mm/dd/yyyy) (Required
field)
Select one of the Gender
radio buttons. (Required
field)
Select one of the Ethnicity
Hispanic/Latino radio buttons. (Required
field)
Note:
Select the Not Specified radio
button if the participant does not disclose his/her ethnicity.
Note:
Check the Not Specified checkbox
if the participant does not disclose his/her race.
Select one of the Individual
with a disability radio buttons to identify if the participant
has a disability. (Required
field)
Select one of the High
School Dropout radio button to identify if the participant is a
high school dropout. (Required
field)
Use the Participant
Type check boxes to select the type of participant. Choose all
that apply. (Required
field)
Note:
If the Other check box is selected,
users must insert the other participant type in the Other
text box.
In the Personal
contact name text box, enter the participants personal contact
name.
In the Personal
contact phone, enter the participants personal phone number. (999-999-9999)
In the Additional
Personal contact name text box, enter an additional personal contact
name for the participant.
In the Additional
Personal contact phone text box, enter an additional personal contact
phone number for the participant. (999-999-9999)
Criminal
Justice Information: This section is Required if the participant
is a Youth or an Adult Offender.
Select one of the Received
pre-release services radio buttons.
Select one of the Pre-release
contact radio buttons.
In the Referral
source text box, enter the participants referral source information.
Select one of the Mandated
Enrollment radio buttons. (Required
if the participant is a Youth or an Adult Offender)
Select one of the Alternative
Sentence radio buttons. (Required
if the participant is a Youth or an Adult Offender)
Select one of the Probation/Parole
radio buttons.
In the Date
of Most Recent Probation text box, enter date of most recent probation.
In the Date
of Most Recent Contact with Probation/Parole Offices text box,
enter the date of the most recent contact with the probation/parole officer.
User can also use the (Calendar)
icon to select the date.
Using the Type
of Contact with Probation/Parole Officer drop-down list, select
the type of contact the participant had with his/her probation/parole
officer.
Note:
If the Other is selected, use
the Other text box to enter the
type of contact the participant had with his/her probation/parole officer.
