Update Service Provider Form

Description: Welcome to the Add Service Provider page. This page displays the Update Service Provider form. This form allows users the capability to Update a Service Provider record in the YouthBuild system.

Update a Service Provider

Complete the following to Update a Service Provider:

• Users can Edit all fields on this form.

• Identify the type of provider by selecting one of the Provider Type radio buttons. (Required Field)

• In the Provider Name text box, enter the name of the provider. (Required Field)

• In the POC First Name text box, enter the first name of the provider's point of contact. (Required Field)

• In the POC Last Name text box, enter the last name of the provider's point of contact.  

• In the POC Phone text box, enter the phone number of the provider's point of contact. (nnn-nnn-nnnn) (Required Field)

• In the POC Phone Extension text box, enter the phone extension (if applicable) of the provider's point of contact.

• In the POC Fax text box, enter the fax number of the provider's point of contact. (nnn-nnn-nnnn)  

• In the POC Email text box, enter the email address of the provider's point of contact.

• Once all required fields have been completed, click the (Save) button. This will display the Successfully Saved Message.

• Click the 'here' link to return to the Service Provider Management page. The updated Service Provider record will display in the Service Provider Overview section.
