Summary  

Description: Welcome to the Summary page. This page displays information pertaining to the participant's case. This page also contains several functions that are used to view, edit and update the participant’s case. The Summary page is divided into two sections. The first section is the User Information section. The second section contains the Case Sub-tabs. The User Information section remains consistent while users are under the Cases tab. Users can navigate through the various Sub-tabs to add necessary information to the participant's case. The Sub-tabs are: Summary, Assessments & IDP, Services, Outcomes, Follow-up, Notes and Exit.

 

Functions: This page is divided into two sections: User Information and Sub-tabs. The following describes these two sections and the functionality contained within each.

 

 

User Information

The first section displays the User Information.

Functions: This section contains the following functions:

  • (edit): This link allows users the capability to update the participant's case information.

  • Participant Alerts: This link allows the users the capability to view the participants alerts.

  • (add new): This link allows users the capability to add a new alert for the participant.

  • Add a New Case: This link allows users the capability to add a new participant case to YouthBuild.

  • Add a New Service: This link allows users the capability to add a new service to the participant's case.

  • Cohort: This field displays the Cohort the participant belongs to.

  • Team: This field displays the current team the participant belongs to.

  • Delete Request: This field displays two links: Add and Edit.

    • When the Add link is active, users can select this link to create a delete request for the selected participant.

    • When the Edit link is active, users can select this link to view the submitted deletion request.

    • Users can also use the Edit link to edit and resubmit a deletion request back to the National Office, if the request was rejected.

  • Status: This field displays the current deletion request status. There are three status that will appear: Blank, Pending, Rejected.

    • If the field is blank, this indicates there is no current deletion request.

    • If the field displays "Pending", this indicates the deletion request has not been Approve/rejected by the Program Office.

    • If the field displays "Rejected", this indicates the Program Office rejected the deletion request.

Sub-tabs

The second section displays the Summary tab. This is one of the Sub-tabs found under the Cases tab. The summary tab is divided into two sections. The first section is the Summary section. This section displays an overview of the participant's case. The second section is the Services/Outcomes section. This section displays those services/outcomes the currently assigned to the participant.

Summary Section

  • The Summary section displays the following:

    • Date of Enrollment: The Date of Enrollment field displays the date the participant was enrolled into the YouthBuild Program.

    • Exit Date: The Exit Date field displays the date the participant exited the YouthBuild program. If the participant has not yet exited the program, this field will be blank.

    • Type of Exit: The Type of Exit field displays the if the participant's Exit was successful, unsuccessful, or if the participant was exited for another reason.

    • Reason for Exit: The Reason for Exit field displays the reason the participant exited the program for another reason. If the participant's exit was a Hard Exit, this field will be blank.  

    • Case Started By: The Case Started By field displays the user whom created the participant's case.

    • Last Note Date: The Last Note Date field displays the last date in which a note was entered in the participant's case.

Services/Outcomes Section

  • The Services/Outcomes section contains five tables: Service, Employment Placement, Education Placement, Certificate/Degree and Arrest/Re-adjudication/Re-incarcerations. If any services/outcomes have been added to the participant's case, users will have the following functions:

  • Assessments & IDP: This tab displays a brief summary of the assessments that have been entered for the participant. This tab also contains information on the various Basic Skills test that have been completed by the participant. The Individual Development Plans (IDP's) that have been set up for the participant are also located on this tab.

  • Services: This tab displays information on the various services and activities assigned to the participant. This tab also allows users access to the Service Provider Management page. From the Service Provider Management page user can create, view, and edit service providers.

  • Outcomes: This tab displays information on the various outcomes pertaining to the participant's employment, education, and arrests/re-adjudications/re-incarcerations.

  • Follow-up: This tab displays the follow-up information pertaining to the participant. This tab will not be visible until the participant has successfully exited the YouthBuild program. If the participant was unsuccessfully exited or exited for another reason, the Follow-up tab will not appear.

  • Notes: This tab displays all notes that have been added to the participant's case.

  • Exit: This tab displays a summary of the participant's exit interview. If an exit interview has not been given, users will have the capability of adding an exit interview. This tab is ONLY visible to the Grantee and Administrator user roles.
